Please answer all questions to the best of your ability and provide all tax documents pertaining to the current tax year. If any question does not apply to you, please write N/A next to the question. More issues and questions might arise during the tax preparation.

Personal information

  • Last 2 years income tax returns (Federal and State, if applicable) if you are a new client
  • Name, address, Social Security number and Date of Birth for yourself, spouse and dependents if you are a new client
  • Copies of your social security cards if you are a new client.
  • Marital status change or change in dependents, if applicable.
  • Do you expect a large fluctuation of your income, deductions, or withholdings? If yes, please explain.
  • Health Insurance Coverage Form(s), if applicable.
  • If you received health insurance subsidy during the year, please provide Form 1095-A.
  • Authorization code received from the Franchise Tax Board if you reside in California.
  • Banking information (Name of bank, type of account, account number and routing number) if Direct Deposit is required

  • Did you engage in a sale transaction involving cryptocurrency (bitcoin)? YES or NO. Please circle or highlight one answer. If yes, we need to discuss.
  • Gambling/Lottery Winnings and Losses/Prizes/Bonus
  • Alimony Income
  • Rental Income and expenses including depreciation schedule. If new property, please provide the date of purchase or conversion from principal residence to rental property. Also, will need the closing statement.
  • Self-Employment income and expenses. For auto expenses, please confirm if you are using a mileage log to keep track of your business miles. If not, we need to discuss.
  • Social Security benefits (Form SSA-1099)
  • Foreign Income, if applicable.
  • Foreign bank accounts, if applicable.

  • Dependent Care Costs (Name, address and EIN of care provider. Also, the amount paid for each child)
  • Education/Tuition Costs/Materials Purchased (Form 1098-T)
  • Medical/Dental Expenses
  • Mortgage/Home Equity Loan Interest (Form(s) 1098)
  • Real Estate Taxes
  • Personal Property Taxes such as DMV fees.
  • Charitable Contributions Cash/Non-Cash. Please note the IRS requirement for a written documentation. Please let us know if we need to discuss.
  • Gambling/Lottery Expenses
  • Did you engage in a purchase transaction including cryptocurrency (such as bitcoin)? YES or NO. Please circle or highlight one answer. If yes, we need to discuss.
  • Did you make any gifts during the year directly, or indirectly, or in a trust, exceeding $15,000 per person? YES or NO. Please circle or highlight one answer. If yes, we need to discuss.
